As a Prevent Blindness Texas volunteer, there is a wide-range of volunteer opportunities available such as becoming a certified vision screener, assisting at special events or participating in other ways that help us achieve our mission of preventing blindness and preserving sight. The Process

The process to apply to become a Prevent Blindness Texas volunteer is as follows:

  1. Please review pages 1-8 of the Volunteer Application Packet and keep for future reference.  
  2. Please SUBMIT and COMPLETE the following forms:
    • Volunteer Application Form
    • Background Screening Consent Form (Please review full version of the End User Agreement Found below)
    • Volunteer Acknowledgement and Receipt
  3. Fax, email, or mail your completed forms to the Prevent Blindness Texas office for which you are applying to become a volunteer.

Once Prevent Blindness Texas reviews your application and completes a background check, you will be contacted to discuss your volunteer interests and availability. If you have additional questions or need additional information, please contact your nearest Prevent Blindness Texas office.  We look forward to hearing from you!
